Almost Home prevents and confronts homelessness in Adams and Weld County through several emergency assistance programs. Each of their programs are designed to make homelessness rare, brief or one time. Their targeted services help members of the community develop the skills and obtain the resources to reach long-term sufficiency.

The Homeless Prevention Program provides Emergency Assistance and short-term case management to households at risk of eviction or foreclosure as well as to households at risk of utility shut-offs.

Almost Home's Supportive Housing Programs provide emergency assistance and ongoing supportive, home-based case management services to help ensure housing stability and long-term self-sufficiency.

Almost Home's Family Emergency Shelter provides assistance to seven families experiencing literal homelessness. While in the emergency shelter families work towards self-sufficiency and securing permanent housing.

Two case managers provide Street Outreach, visiting known encampments and other areas in order to offer services to individuals.

Almost Home's Severe Weather Activation Plan (SWAP) provides motel vouchers to households experiencing literal homelessness during periods of severe weather. In addition, all clients receive supportive case management and re-housing assistance.
