Stated Purpose:
Achieve collective impact to generate health as a foundation of thriving communities by expanding access, improving quality, eliminating disparities, enhancing care experience, controlling costs, managing care, and creating a culture of health.
Year, State Incorporated:
2002, CO
The North Colorado Health Alliance community joint venture is built upon the foundation principles of engaging community, forging partnerships, aligning resources, integrating care, supporting innovation, and creating a culture of health. Alliance partners and agencies share the vision of a Thriving Colorado. Individuals and organizations at the local and regional level share a community health improvement plan, brought to life through the Make TODAY Count! campaign, that guides and promotes a better understanding of and action on the complex of factors that contribute to the health of communities. The ongoing integration of health and other community services with the constant innovation and willingness to take on diverse projects reflect its widening understanding of health as a single puzzle with many interlocking and equally indispensable pieces. The Alliance manages care for Medicaid patients enrolled in Colorado’s Regional Collaborative Care Organizations; does outreach and enrollment for Colorado’s health insurance exchange and Medicaid; promotes healthy eating, active living, general wellbeing and community progress toward Healthy People 2020 goals; manages care for at-risk vulnerable pregnant women; sustains a scalable community health data dashboard; and uses data to inform its shared community strategies.
