Legacy Community Health was formed when two health service organizations—the Montrose Clinic and The Assistance Fund—merged in 2005. Patients and clients of both agencies, each of which had been serving Houston since 1981, now had a single, accessible place to turn for care and services. In the ensuing 40 years, Legacy has provided care services to Southeast Texas in a culturally sensitive and judgment-free environment. A United Way-affiliated agency since 1990, we currently provide adult primary care, pediatrics, geriatrics, dental care, vision services, behavioral health services, OB/GYN and maternity, vaccinations and immunizations, health promotion and community outreach, wellness, and nutrition services for Texans who have traditionally faced barriers to care. In addition, we specialize in HIV/AIDS testing, education, treatment and social services, as well as caring for other chronic health conditions like diabetes and high blood pressure. Legacy is the largest Federally Qualified Heath Center (FQHC) in the state of Texas, with 54 total clinic locations across Houston, Beaumont, Deer Park, and Baytown. We also operate school-based clinics on KIPP, Yes Prep, and Galena Park ISD campuses. Legacy served 149,199 patients in 2021, an 11% increase over 2020. More than 92% of our patients live at or below 200% of the Federal Poverty Line, and only 19% use a form of private insurance to pay for their care.
