Youth & Family Connections (YFC), is a 501 (c)(3) community based service organization that has been in operation since January 2006 serving youth throughout Weld County. Its mission for the past eight years has been to provide coordinated solutions to youth engaged in high risk behavior, which may include criminal conduct, substance abuse, chronic run away, pregnant/parenting teens, truancy and defiant youth who are out of their parent or guardians control. Through a blend of prevention and intervention strategies, comprehensive assessments, and coordinated case management support, Youth & Family Connections staff actively engage families and key partner agencies in the community to collaboratively identify ways to connect youth and families to the direct services they need, giving them multiple options for success.

Its goals are to help youth remain in school, graduate with a high school diploma or equivalent educational certificate; and to keep youth from entering or further penetrating into the Juvenile Justice System.  Since January 2010, it has operated a satellite office in south Weld County, now located in Ft. Lupton, to better serve children and families in that area of the county.
