Stated Purpose:

ChooseWell Communities equips families in recovery with children under the age of five to choose well for their futures. The nonprofit's focus is on accompaniment and support for multiple years as families are re-crafting their lives, committing to sobriety, and rearing their young children.  More specifically, it supports (1) access to subsidized housing vouchers that are awarded to families committed to a healthy home without substance misuse through their childrens’ fifth birthdays, and  (2) pathways for educational or vocational success that are established and nurtured based on each parent’s strengths and interests.

ChooseWell’s primary program is Housing First program for young families called Project Thrive. Rebuilding their lives after successfully completing an addiction treatment program, families are provided access to safe, affordable housing and for multiple years are surrounded by a nurturing, supportive community that addresses the challenges to new sobriety: purpose and vocation, childcare, transportation, and behavioral health services. In 2024 ChooseWell added a pilot program called KYnship Creates funded via a Louisville Metro Government Opioid Settlement Grant. It is an innovative pilot program that adds a workforce development framework to the original housing first support model championed by ChooseWell Communities.
